DO YOU OR YOUR FAMILY NEED ASSISTANCE?
If so, the Wayne Goodfellows are here for your support.  The first step is to make sure you qualify for the assistance.The purpose of this assistance is to provide short term, emergency aid to individuals whom reside in the  City of Wayne and have had a catastrophic event occur in their lives which prohibits them from providing for themselves in the short term. (i.e. house fire, flood & etc.)If you feel you do qualify for this assistance, please completely fill out the form below that applies to your situation and you MUST include the following information:

1.)  Valid Driver’s License
2.)  Utility bill or other bill dated within last 30 days of application address must match the
address on the driver’s license.
3.) LAST YEARS INCOME TAX RETURN. Return MUST have children listed on it. ONLY
children listed on return will qualify, unless they are newborn.
4.) Provide Proof of Children’s AGES. 14yrs and UNDER will receive assistance. State
Aide paperwork with children listed with ages or report cards.
